Lauren Collen, MD: Advanced Combination Therapy May Be Effective Option for Pediatric Refractory IBD

As the incidence and prevalence of pediatric IBD increases, so does the increased number of pediatric patients who do not respond to monotherapy with a biologic or small molecule inhibitor. Investigators sought to examine the efficacy and safety of advanced combination therapy in pediatric patients with IBD. They conducted a single-center, retrospective study of 12 patients aged < 18 years with an IBD diagnosis—either Crohn’s disease (n = 7), ulcerative colitis (n = 3), or IBD unspecified (n = 2)—with a primary outcome of steroid-free clinical remission at 6 months. Participants were included if they failed ≥ 1 biologic agent or molecule inhibitor as monotherapy and continued an advanced combination therapy for ≥ 4 months.

The mean duration of the first trial of advanced combination therapy, a treatment involving TNF-alpha inhibitors, vedolizumab, ustekinumab, dupilumab, and JAK inhibitors, was 27.1 months. Among the 12 patients, 7 (58.3%) achieved steroid-free clinical remission at 6 months of advanced combination therapy.

“The therapeutics that we're using, anti TNF, il 23 blockade il 1223, blockade Jak inhibitors, anti-integrins, there are all, for the most part, targeting specific immune signaling pathways,” said investigator Lauren Collen, MD, from Boston’s Children’s Hospital during an on-site interview with 

 “I think for some patients with IBD, and in particular refractory IBD, there might just be more than one kind of dominant aberrant signaling pathway at play, and I think some of these patients just need to target more than one pathway to get their disease into remission.”

Out of the 5 patients who did not achieve clinical remission at 6 months, 1 patient achieved SFCR at 12 months, 2 patients switched to an alternate advanced combination therapy regimen, and 2 patients discontinued ACT and returned to monotherapy or surgery.

The study also showed 9 patients on advanced combination therapy (75%) demonstrated normal CRP at 6 months. Among the 6 patients who had extraintestinal manifestations of IBD, 4 (66.7%) demonstrated remission of EIMs at 6 months. Investigators observed no adverse events.

Collen recognized the challenge of advanced therapies in pediatric populations. In studies, they often examine off-label use drugs, approved by the US Food and Drug Administration for adult use, for pediatric inflammatory bowel diseases. However, this creates an insurance obstacle since insurance companies are resisting to cover drugs for children that are not FDA-approved, and therefore this can delay treatments for children.

“When you have a patient who has often failed many biologic or small molecule drugs when they're used individually…you really feel like they're a patient that needs 2 drugs to target….2 different mechanisms to get their disease and remission,” Collen said. “It is an even added burden because often you're requesting multiple drugs that aren't FDA approved, and it makes for challenges getting approval and getting insurance companies to buy in. The pediatric IBD community would really benefit from more FDA approvals and better access to the drugs that we know work in adult IBD, and there's tons of retrospective data to show that it works in kids, too. We just need easier access to those drugs.”

Analysis of more than 2 dozen studies in biliary atresia provides an overview of serum values as prognostic indicators for native liver survival following hepatic portoenterostomy.

 (NASPGHAN) 2024 annual meeting, results detail the prognostic value of several serum values, including total bilirubin (TB), ALT, GGT, MMP-7, and total bile acids (TBA).

“My goal was to create a uniform algorithm we can say, as clinicians can follow using simple to use serum markers that are either old or new to prognosticate at different time points” explained Ahmad Anouti, MD, of the Digestive and Liver Disease, The University of Texas Southwestern Medical Center, in an interview with 

Despite being the most common treatment for biliary atresia, which is the leading cause of pediatric liver transplantation, most patients will still go on to require transplant, with a considerable degree of variation observed for time until transplant among this group. According to an October 2023 study by Anouti, 2754 biliary atresia waitlist editions and 2206 biliary atresia liver transplants were recorded within the UNOS database from January 2010 through December 2021.

To better inform management strategies, Anouti and fellow investigators designed a systemic review and meta-analysis to elucidate associations between serum values and native liver survival among patients with biliary atresia. The study leveraged data from the PubMed, MEDLINE, SCOPUS, and EMBASE databases published from 1946 through December 2023 examining the prognostic values of serum values for biliary atresia after hepatic portoenterostomy.

The specific values of interest for the study were TB, ALT, GGT, MMP-7, and TBA. For the purpose of analysis, non-function hepatic portoenterostomy was defined as persistent jaundice, cirrhosis, liver transplantation, or death. Investigators pointed out associations between pooled serum variables with native liver survival were calculated as hazard ratios (HRs) and odds ratios (ORs) using the inverse variance weighting method.

From their search, a total of 30 studies were identified for inclusion. From these studies, investigators obtained data from 4399 BA patients, including 2073 with successful hepatic portoenterostomy and 1793 with non-functioning hepatic portoenterostomy. Investigators noted the mean time to hepatic portoenterostomy for the success group was 63.3 (SD, 0.18) days compared with 69.5 (SD, 0.20) days in the failure group (

Investigators spotlighted a single study suggesting non-liver-specific survival decreased when MMP-7 levels exceeded 10.3 ng/mL (HR, 4.29; 95% CI, 1.12 to 16.52). Disparities in use of biologic medications such as dupilumab and tralokinumab for 

 have been identified in an analysis of 50 countries, with barriers shown to be related to sociodemographic status.

These findings resulted from an analysis of the association between burden of atopic dermatitis, also known as eczema, and sociodemographic index (SDI). The investigators were led in part by Arbie Sofia P. Merilleno, from the Research and Innovation Institute and the department of medicine at Women's College Hospital in Toronto, Canada.

“Biologics have transformed the treatment of moderate-to-severe atopic dermatitis, but new treatments are expensive and not universally accessible,” Merilleno and colleagues wrote. “The objective of this study was to describe global utilization patterns of dupilumab and tralokinumab from 2017 to 2022 and estimate the associations of country-level economic status and atopic dermatitis disease burden with biologic utilization.”

The research team implemented pharmaceutical sales data from IQVIA MIDAS for the period between 2017 - 2022, looking into international trends in biologic medications for eczema across countries by examining the volume and value of sales of such drugs at the country level. The team also incorporated the SDI of each of the countries they evaluated, as well as disability-adjusted life years (DALY) and prevalence rates for eczema.

The latter set of data was sourced by the investigative team from the Global Burden of Disease Study, and they also looked at population data drawn from the World Bank. Biologic implementation rates were calculated by the investigators for each year by dividing the total units sold by the country’s population. They placed the rates into categories which were according to the SDI groups (low-middle/middle, high-middle, and high) and charted the annual averages.

For the purposes of exploring how SDI and atopic dermatitis DALY rates were connected to use of biologics, the research team used both univariable and multivariable linear regression analyses. In their sensitivity analysis, the team looked at prevalence rates rather than DALY rates to assess any link with biologic utilization in a univariable linear regression.

Any possible impacts of the COVID-19 pandemic were accounted for by the investigators when they conducted additional analyses through the use of data only from 2017 - March 2020.

The research team concluded that, over time, the global biologic use for patients with atopic dermatitis climbed significantly, especially in countries found to have high SDI scores. They found that biologics were only purchased in 3 high-SDI countries in 2017, specifically the US, France, and Germany, and the United States.

The investigators added that in these countries, the average biologic use rate was shown to be 1.1 units per 100,000 individuals. They also noted that by 2022, use of biologics had expanded to 47 more countries, leading to an average increase in use of 175.4 times, growing from 1.1 units per 100,000 in 2017 to 192.9 units per 100,000 individuals in the year 2022.

In 2022, the investigative team also found that countries in the high-SDI category showed an average rate of biologic use that was 28.2 times greater (329.6 units per 100,000) than that of lower-middle/middle-SDI countries (11.7 units per 100,000).

SDI was reported by the team to be heavily correlated with use of biologics, the team’s multivariable linear regression suggested. In fact, they highlighted that nations with a high-middle SDI utilized 10.9 more medication units (95% CI; −87.6 to 109.5). Additionally, high-SDI countries were shown by the investigators to have used 118.0 more units (95% CI; 27.7–208.3) per 100,000 individuals versus those with low-middle/middle-SDIs.

Despite such findings, the research team noted a lack of clear link between biologic usage and atopic dermatitis burden assessed by DALY rates, adding that there had only been a 0.7-unit rise in biologic implementation per 100,000 patients for each 1 DALY increase per 100,000 (95% CI; −0.3 to 1.7). In a similar vein, the team highlighted a lack of connection identified between rates of eczema prevalence and biologic use.

These data were shown to have held steady even when data following March 2020 was excluded from the analysis.

“This study highlights disparities in biologic utilization for atopic dermatitis between countries related to their sociodemographic status,” they wrote. “This relationship is most likely driven by the high costs of biologics restricting access in low-middle and middle-SDI countries. While biologics are available in some Latin American countries, financial barriers often limit their use.”

Early findings of a preliminary analysis indicate some plastics and fragrances may be more prevalent in the exposomes of patients aged 0 – 18 years with 

, compared with age-matched controls. However, since this study is still in the early stages of research with only a small sample size, the findings are not statistically significant.

Investigator Lauren Collen, MD, from Boston Children’s Hospital, talked about the early data in an interview with 

 Meeting in Hollywood, Florida, from November 7 to 9, 2024.

“I think the initial step here is to understand the big picture, what some of the environmental triggers of inflammatory bowel disease are, and then certainly down the line, therapeutics or preventative measures might target those specific exposures, but I think we're a couple steps away from that,” Collen said.

The incidence of various inflammatory bowel disease (IBD) subtypes, including Crohn’s Disease, has increased in early life over the past 2 decades. However, with only 15% of patients having monogenic etiologies for very early onset IBD, the disease increase cannot solely be explained by genetics—the environment plays a role, too.

Environmental exposures influence an infant’s immune response and can increase susceptibility to immune-mediated diseases. Biological, chemical, physical, and psychosocial factors all come together to create an exposome. Previous research has evaluated the association between Crohn’s disease and individual environmental risk factors, such as pathogens, pharmaceuticals, psychological stress, diet, and chemicals, but the results were inconclusive.

Collen and colleagues are conducting a prospective study to evaluate the role of environmental influences on Chrohn’s disease susceptibility by examining exosomes. They conducted an exposomic tool, the Fresh Air sampler, that could comprehensively capture personal exposures to chemical contaminants from air, water, consumer products, combustion activity, building materials, furnishing, clothing, diet, and pharmaceutical sources. Investigators used this tool with high-resolution mass spectrometry to analyze the occurrence and distribution of thousands of environmental chemicals.

In July 2022, the team recruited 180 patients recently diagnosed with Crohn’s disease and 180 sex-and age-matched controls through the VEO-IBD Consortium at Boston Children’s Hospital, The Hospital for Sick Children, and the Children’s Hospital at Ludwig Maximilians-University of Munich. The exposome was characterized in 45 patients.

The Fresh Air sampler, worn on the ankle for patients < 3 years and worn on the wrist for older patients for 5 days, detected exposures from various locations, such as school, home, and outdoors. The sampler contains a polytetrafluoroethylene case that houses polydimethylsiloxane (PDMS) sorbent bars that collect airborne contaminants.

The samplers demonstrated participants were exposed to 5954 features, including oxygenated alkanes, as well as aromatic and oxygenated hydrocarbons. A Cheminformatics analysis identified 58 endocrine disrupters and 67 chemicals with predicted genotoxicity, such as phthalates, benzoates, and salicylates, in exposures. The study also showed most exposed chemicals were antimicrobials, preservatives, colorants, and flavorant in personal care, household care, and cleaning products.

Colleen said the team is looking for differences in exposures such as fragrances, pesticides, smoke, pollution, furnishings, and food additives. With the study still ongoing, the team aims to recruit ≥ 300 more patients.

“We're still recruiting at 6 different sites, and so we're excited to engage with patients with recently diagnosed Crohn's disease,” Collen said. “We're hoping to get more patients to wear these bracelets so that we have more data to analyze and present at a future meeting.”

Preliminary analysis suggests increased prevalence of plastics and fragrances in the exposomes of pediatric Crohn’s disease patients compared to controls, though findings lack statistical significance due to small sample size. The study, led by Dr. Lauren Collen, aims to identify environmental triggers of inflammatory bowel disease (IBD) using the Fresh Air sampler to capture chemical exposures. Initial results show exposure to numerous environmental chemicals, including endocrine disruptors and genotoxic agents. The ongoing study seeks to recruit more participants to enhance data robustness and potential therapeutic insights.

Lauren Collen, MD: Some Fragrances May be More Prevalent in Exposomes  of Children with Crohn’s Disease

Use of linaclotide (Linzess) offers significant benefits for relief of 

 among children with neurodevelopment disorders, according to new research.

An analysis of the pivotal phase 3 trial supporting approval from the 

 (NASPGHAN 2024), results of the analysis demonstrate use of linaclotide provided similar benefits among children with neurodevelopmental disorders observed in the overall trial.

“So, the idea here is this is a unique population that we really want to better understand how to best manage their constipation in all forms, but with the introduction of linaclotide, the first FDA-approved treatment for pediatric constipation, I think we wanted to do a little bit of an in-depth analysis, given those specific concerns of those patients. That's what prompted this posthoc analysis,” explained lead investigator Hannibal Person, MD, assistant professor and medical director of the Center for Diversity and Health Equity at the University of Washington, in an interview with HCPLive at NASPGHAN 2024.

 as the first therapy to receive approval for pediatric patients with functional constipation, linaclotide has quickly found its way into treatment algorithms for this patient population. The approval, which was awarded to Ironwood Pharmaceuticals, was based on data from a double-blind, placebo-controlled, randomized, multicenter phase 3 clinical trial.

The study enrolled 330 patients aged 6 to 17 years with functional constipation and randomized them in a 1:1 ratio to oral linaclotide 72 μg or placebo once daily for 12 weeks. Results of the overall trial suggested use of linaclotide was associated with significant improvement in the weekly spontaneous bowel movement frequency relative to placebo therapy during the 12-week treatment period (

At NASPGHAN 2024, Person presented data from an analysis examining outcomes among patients with functional constipation and comorbid neurodevelopment disorders, including aggressive behavior, ADHD, ASD, conduct disorder, disruptive mood dysregulation disorder, oppositional defiant disorder, sensory processing disorder, and tic disorders. Investigators pointed out were reported by the child's parent or legal guardian and corroborated by medical records when available.

“I think most general pediatricians and pediatric gastroenterologists have experienced sort of the difficulties these families face with managing functional constipation in kids with developmental disorders, and that can be really multifactorial,” Person explained.

Among the 328 patients in the study who received at least 1 dose of linaclotide or placebo therapy, 57 had a comorbid neurodevelopmental disorder. Of the 57 patients included in the current analysis, 25 had received linaclotide and 32 had received placebo therapy. Investigators noted baseline demographics were generally balanced between treatment groups.

Upon analysis, results indicated use of linaclotide was associated with a beneficial treatment effect for all efficacy endpoints, with the observed improvements in bowel function similar to those observed in the overall study population. Analysis of adverse events suggested the incidence of diarrhea treatment-emergent adverse events for children with functional constipation and comorbid neurodevelopmental disorders (linaclotide, 4.0% vs placebo, 0%) was similar to that observed in the overall study population (linaclotide, 4.3% vs placebo, 1.8%), which Person highlighted is a similar rate seen with laxatives and other modalities.

Further analysis drew attention to the rates of compliance among this patient population. In the analysis, results demonstrated approximately 96% of patients completing 8–12 weeks of treatment. Furthermore, patients in this subgroup achieved 96% or greater dosing compliance with once-daily oral administration of linaclotide or placebo therapy.

“The fact that the kids did well, just like their peers, was a really great sign,” Person added. “The other thing is the rate of adherence to the medication, which can be challenging with any child, but sometimes particularly challenging with this population—we had a very high rate, greater than 90% of adherence for 8 weeks of treatment, which was fantastic.”

Linaclotide (Linzess) has shown significant efficacy in treating functional constipation in children with neurodevelopmental disorders, as demonstrated in a phase 3 trial.
